The deadline to submit the claim form is June 3, 2026.
Before submitting a claim:
- Submit one form per occupant.
- Prepare scanned documents (photo, PDF, or JPEG) to upload in support of your claim.
- The administrator requires these documents to identify you. The data will be stored on a secure site and destroyed by the end of 2026.
Proof of identity – Adult claimant
Two (2) government‑issued photo ID documents, for example:
- Québec Health Insurance Card
- Passport
- Driver’s licence
Proof of identity – Children (adult or minor) making a claim
A birth certificate is mandatory, as well as one of the following:
- Health Insurance Card
- Passport
- Driver’s licence
- School report card
- Childcare expense statement (R24)
For non‑occupant owners
A copy of the lease or a sworn declaration is required.
For tenants
A copy of the lease, an R31 statement, or a utility bill such as:
- Hydro‑Québec bill
- Telecommunications services bill
